Breaststroker's Knee

In swimming, one of the more common injuries is Breaststroker's Knee, an overuse injury largely due to the kick needed for the stroke. 

What is Breaststroker's Knee?

Breaststroker's Knee is a combination of a strain to the medial collateral ligament (MCL) and compression on the lateral knee, seen largely in response to the kick performed in breaststroke. The breaststroke kick is a high valgus load produced during sudden flexion-extension, adduction and external rotation of the knee against the hydrodynamic environment, resulting in stress to the medial compartment.

As a result, it is not uncommon for swimmers to present with ligament sprains on the MCL, irritation of the medial plica and bursal irritation at the muscular insertions of the adductor and hamstring muscles. Furthermore, strain injuries can be present in the adductor muscles (adductor magnus and brevis, especially)

Signs and Symptoms of Breaststroker's Knee

Symptoms of Breaststroker's Knee include: 

  • Pain in and around the knee
  • Knee pain that worsens with exercise
  • Swelling
  • Reduced mobility around joint

Causes of Breaststroker's Knee

Breaststroker's Knee is largely due to the overuse of the knee in the breaststroke kick. This is especially true if the technique used isn't correct, or after intense or prolonged sessions of kicking training.
